Eligibility and Enrollment
You may enroll in County-Provided Voluntary Group Term Life Insurance only within 31 days from the date of eligibility, or during annual Open Enrollment. Your coverage will take effect the first of the month following hire date. Please review your benefit options carefully. Employees who do not enroll in benefits within 31 days of their eligibility date may be subject to other restrictions and/or reduced benefits.
What does this program offer you?
Guaranteed Coverage: You may choose any coverage amount, in $10,000 increments, to a maximum of 5 times your annual salary or $500,000, whichever is less. Coverage is guaranteed to $250,000 or 5 times your annual salary (if less) regardless of your health condition if you enroll during your initial eligibility period. Coverage requests over $250,000 are subject to approval by the carrier. The minimum amount of coverage is $10,000.
Spouse Coverage Options: You may choose coverage in $5,000 increments for your spouse. The maximum amount of coverage is 50% of the amount that you selected for yourself or $125,000 whichever is less. Spouse coverage is guaranteed to $75,000 if you enroll during your initial eligibility period. Amounts selected above $75,000 are subject to approval by the provider prior to becoming effective. Employee must be enrolled in Voluntary Group Term Life Insurance to elect spouse coverage.
Dependent Coverage Options: Coverage of $5,000, $10,000 or $20,000 is also available for each of your dependent children from birth to the end of the calendar year in which child turns 25. If you and your spouse are employed by Johnson County Government, only one of you may elect coverage for your children. You and your spouse may elect to be covered as an employee or as a dependent, but not both. You are automatically the beneficiary for your spouse's and dependent’s coverage. Employee must be enrolled in Voluntary Group Term Life Insurance to elect dependent coverage.
How much will your coverage cost?
Employee Coverage and Costs: You can figure your monthly premium payment easily by using the schedule contained in your enrollment materials. Simply locate the amount of coverage on the chart under the employee’s age bracket. (employee's age as of January 1 each year is used to determine the rate for both employee and spouse).Program Changes: You will be able to make changes to your election during the Benefits Open Enrollment each year. Changes due to changes in family status must be requested within 30 days of the event and must be consistent with that change in family status.
