Qualified Johnson County Government employees may also purchase voluntary short-term disability coverage through CIGNA Insurance Company.

Eligibility and Enrollment
You may enroll in Voluntary Short-Term Disability only within 31 days from the date of eligibility or during annual Open Enrollment. Full-time, regular employees who are U.S. or Canadian citizens or residents are eligible for this program. Please review your benefit options carefully. Employees who do not enroll in benefits within 31 days of their initial eligibility date may be subject to other restrictions and/or reduced benefits.

Plan Features
> Benefit: 60% of the first $1,250 of weekly pre-disability earnings, reduced by deductible income
> Maximum short-term disability benefit: $750/week, before reduction by deductible income
> Minimum short-term disability benefit: $15/week
> Benefit waiting period: 14 days for a disability caused by injury, sickness, or pregnancy. Must be a non-occupational disability
> Maximum benefit period: 180 days minus the length of your benefit waiting period.

Benefits Cease
> The date you are no longer disabled
> The date your Maximum Benefit Period Ends
> The date you die
> The date benefits become payable to you under any other disability insurance plan under which you become insured through employment during a period of temporary recovery
> The date you fail to provide proof of continued disability and entitlement to short term disability benefits

Exclusions and Limitations
You are not covered for a disability caused or contributed to by any of the following: 1) intentionally self-inflicted injury, while sane or insane; 2) a disability arising out of or in the course of any employment for wage or profit; 3) war, or any act of war; 4) loss of your professional license, occupational license, or certification; 5) committing or attempting to commit an assault or felony, or actively participating in a violent disorder or riot.

Late Enrollment Penalty
If enrolling after your initial eligibility period, you will be subject to a late enrollment penalty. If you file a claim for anything other than an accidental injury during the first 12 months, your benefits will be payable after a 60-day waiting period. After 12 months of coverage, the late enrollment penalty no longer applies, and the waiting period reverts to the 14-day elimination period.

Benefits are not payable
> For any period when you are not under the ongoing care of a physician
> For any period when you are confined for any reason in a penal or correctional institution
> For any period when you are eligible to receive benefits under a workers' compensation law or similar law
> For any period of disability when you are able to work in your own occupation and able to earn at least 20% of your indexed predisability earnings but you elect not to work
> For any period of disability when you are not participating in good faith in a plan, program, or course of medical treatment or vocational training or education

Cost
The cost of Voluntary Short-Term Disability is based on the employee's age and weekly income. Please refer to the separate leaflet (available from OFM-Benefits) for rates and calculation formula.
