Vision Service Provider (VSP) Information 

How to Make an Appointment with a VSP Provider:
> Find a VSP participating doctor by calling 1-800-877-7195 or online at www.vsp.com.
> Call the doctor to schedule your appointment, and advise your doctor that you are a VSP member. You will also need to provide your social security number, employer name, and plan number (#12 062509 0002 0003). Your doctor will call VSP to verify your eligibility, plan coverage, and obtain authorization for services/materials.
The doctor will inform you at this time if you are not eligible for benefits.

Obtaining Services With a Non-Participating Provider:
> Pay the full amount of the bill at the time service is provided.  
> Submit a claim for reimbursement to: Vision Service Plan, Attention: Non-Member Claims, P.O. Box 997100, Sacramento, CA 95899-7100.

The claim must include the following information: itemized receipt, employer name, employee name, mailing address, employee social security number, patient's name, and patient's date of birth.
